We offer wellness plans to help provide your furry family members with all of the preventive care they need for an entire year! Signing up on a wellness plan takes the guesswork out of your pet’s preventive care to keep them as healthy as possible and it helps make your pet’s care more affordable over time! It’s a win-win situation!
In addition to the included vaccines and lab work, you will also get unlimited free office visits for the entire year your pet is on the wellness plan, and you also get a 5% discount on MOST other services provided to your pet.
We offer two different level plans depending on the age of your pet: Kittens and puppies (under 1 year old) can decide between the complete plan that includes a spay/neuter and microchip in addition to their regular preventive care or the basic level plan that includes their regular preventive care only.
Adult cats and dogs (over 1 year old) can decide between the complete plan that includes an anesthetic dental cleaning, blood pressure evaluation, ECG, and microchip in addition to their regular preventive care or the basic level plan that includes their regular preventive care only.

These conditions, once agreed upon, constitute a Wellness Plan Agreement and will be referred to as the “Agreement”.
BETWEEN:
Cinco Ranch Pet Vet Wellness Clinic DBA Long Meadow Veterinary Clinic of 7909 W. Grand Pkwy S. #230, Richmond, TX 77407(the “Clinic”) AND
You the legal owner of the animal listed in this Agreement (the “Client”)
COMMENCEMENT DATE AND TERM:
The Agreement will take effect on the day the Agreement is signed and the Client’s initial payment is made to the Clinic (the “Effective Date”) and shall remain in effect for 12 months (the “Term of the Agreement”).
AUTO-RENEWAL AND AUTO-ESCALATION:
On the day the Term of the Agreement is set to expire, the Agreement will automatically renew for a new 12 month Term of the Agreement (the “Renewal Term”) until it is terminated by either party in writing or the Client elects in writing to enter into a new agreement offered by the Clinic that replaces this Agreement.
If the Agreement is specific to the animal’s age, the Agreement will automatically renew for a new 12 month Term of the Agreement (the “Renewal Term”) as a new Agreement that is the closest equivalent Agreement to the current Agreement but appropriate for the animal’s new age and the cost of the plan will be automatically updated to reflect the current cost of the new Agreement.
The automatic renewal re-establishes the Agreement between the Client and the Clinic for the Renewal Term and the date of the automatic renewal is the Effective Date for the Agreement.
Each Renewal Term constitutes a new Agreement under these same conditions unless written notification is provided to the Client by the Clinic of changes to the Agreement or to the Clinic by the Client of cancellation at least 15 days prior to the Renewal Term Effective Date.
INCLUDED PRODUCTS OR SERVICES:
The Client understands and consents to the medical care, products, and services included in the Agreement (the “Included Items”).
Any Included Items may be declined at the request of the Client or on behalf of the Clinic at the discretion of the doctor based on the patient’s prior healthcare, health status, behavioral status, lifestyle and other factors. Any care included on the plan that is not necessary or appropriate for the specific patient will not be provided to that patient. The decision of what care is medically necessary and appropriate is at the sole discretion of the doctor treating the patient acting on behalf of the Clinic.
The Clinic reserves the right to refuse to provide certain Included Items if the patient is not current on basic wellness care including vaccinations (Rabies, DA2PP, Lepto, Bordetella, FVRCP) and a heartworm test. Specifically a patient will be ineligible for the following care that may be included on a wellness plan IF the patient is not current on basic wellness care: Nail trims, Bathing, Technician Visits, Spay Procedures, Neuter Procedures, and Oral Care Procedures.
Any Included Items in the Agreement that are not used during the 12 month Term of the Agreement for any reason are not transferable and are non-refundable.
Any Included Items provided or performed before the Term of Agreement are not relevant to the Agreement.
AGREEMENT PRICE AND PAYMENT:
The Client understands and agrees to pay the total price listed for this Agreement including any Enrollment Fees as stated in the electronic enrollment process.
The Client may select to pay the total price listed as one full payment on the Effective Date of the Agreement OR as pre-authorized, automatic charges on a quarterly or monthly basis (the “Scheduled Payments”) to the Client’s credit or debit card stored on file with the Clinic (the “Card on File”).
CHANGES TO AGREEMENT PRICE OR INCLUDED ITEMS:
The Clinic reserves the right to adjust Scheduled Payments, Included Items and the terms and conditions of this Agreement at any time. The Clinic will give the Client notice of any such changes no less than thirty (30) days prior to the date the changes are to become effective.
LONG MEADOW VETERINARY CLINIC’S CREDIT CARD AUTHORIZATION AGREEMENT
If the Agreement is not paid in full on the Effective Date, the Client authorizes the Clinic to charge the Card on File for Scheduled Payments for the cost of the Agreement until cancellation pursuant to the cancellation terms below. If Scheduled Payments will be made, the date each Scheduled Payment is due will be known as the “Billing Date.”
I authorize the Clinic to charge the Card on File according to the terms outlined. If the Billing Date falls on a weekend or holiday, the Client understands that the Scheduled Payment may be executed on the next business day. The Client understands that this authorization will remain in effect until the Client cancels the Agreement in writing, and the Client agrees to notify the Clinic in writing of any changes to the Card on File account information or cancellation of this Agreement at least 15 days prior to the next Billing Date. This payment authorization is for the Agreement selected during electronic enrollment.
The Client certifies that the Client is an authorized user of the Card on File and that the Client will not dispute the Scheduled Payments with the Client’s financial institution provided the payments correspond to the terms indicated in this authorization form.
In the event the Card on File is declined or fails to successfully process through a Scheduled Payment, the Client authorizes the Clinic to reattempt to process the Scheduled Payment using the Card on File, Clinic account credits or other means of payment at the Clinic’s discretion. The Client also agrees to pay a Reprocessing Fee of $20.00 which will be added to each Scheduled Payment that requires a reattempt to process due to any reason a payment is unable to be processed on the initial attempt based on the account information provided by the the Client to the Clinic.
GENERAL TERMS OF USE:
The Client attests that they are a legal adult over 18 years of age and the legal owner of the animal listed in the Agreement, and thereby legally able to enter into and be bound by the Agreement.
The Agreement and the Included Items are not transferable to another animal or assignable to another person. The Agreement applies only to the animal designated on the Agreement while owned by the Client who entered into the Agreement.
Any Included Items in the Agreement that are not used within the twelve (12) month Term of the Agreement are not transferable to any Renewal Term and are non-refundable.
The Client agrees and understands that the Included Items are NOT insurance but a package of specific medical care, products and services. The Agreement is not a substitute for pet insurance and will not cover any care at the Clinic that is not explicitly listed in the Agreement and will not cover any care, products or services at veterinary facilities other than the Clinic.
The Client agrees that any medical care, products, or services approved by the Client and provided by the Clinic that are not included in the Agreement must be paid in full to the Clinic at the time they are rendered and cannot be added to the Agreement.
EXCLUSIONS:
The following items are specifically excluded from this Agreement:
Any Unscheduled Visit: Emergency Visits, Work In Visits, Late Arrivals, Walk In Visits
Specialty Vaccinations (Lyme Vaccine, Rattlesnake Vaccine, etc)
Additional other medical care, products or services recommended by veterinarians at the Clinic that are not explicitly included in this Agreement.
Any care, products or services at any veterinary care facilities other than the Clinic including but not limited to specialists or other veterinary clinics which the Clinic may refer the Client to from time to time.
DEFAULT:
In the event the Client’s Scheduled Payment fails, the Clinic will refuse to provide ANY medical care, products and services, including those NOT included in this Agreement, until payment of all outstanding Scheduled Payments and Reprocessing Fees is complete.
In the event the Client fails to pay an outstanding Scheduled Payment and any Reprocessing Fees within thirty (30) days of its Billing Date, the Clinic has the option to immediately cancel this Agreement, refuse to provide additional medical care, products and services, and declare all Included Items rendered to date be paid in full immediately at current retail price without any discounts. At this time or anytime thereafter, the Client’s account may be referred to a third party collection agency, and the Agreement will not be eligible for reinstatement. Collection activity may negatively impact the Client’s Credit Bureau information and is subject to additional collection fees.
CANCELLATION:
Either the Client or the Clinic may cancel this Agreement at any time, but there may be monies due upon cancellation.
Cancellation by Client Within Three (3) Business Days of the Effective Date: If Client cancels this Agreement for any reason within three (3) clinic working days of the Effective Date, the enrollment fee and any other monies paid toward the cost of this Agreement are refundable less the undiscounted retail value of any and all medical care, products, and services provided.
Cancellation by Client After Three (3) Business Days: If Client cancels for any reason (including, but not limited to death, loss, or transfer of ownership of the Animal) at any time more than three (3) clinic working days after the Effective Date, the enrollment fee is non-refundable. The Clinic will also be entitled to retain or recover from Client all Scheduled Payments that have previously been paid or become due, including any Scheduled Payment for the month or quarter in which cancellation occurs, regardless of the date the cancellation request is submitted. In addition, if the Client requests cancellation prior to the end of the Term of Agreement, the Clinic is entitled to retain or recover from the Client total undiscounted retail value of the Included Items provided less the sum of Scheduled Payments collected during the current 12 month Term of the Agreement. The Client shall be obligated to do the least costly of the following:
-Immediately pay full, undiscounted retail fees for all Included Items provided less the total Scheduled Payments received by the Clinic during the current Term of the Agreement: OR
-Immediately pay in full the total for all remaining Scheduled Payments due under this Agreement for the current Term of the Agreement.
Cancellation by the Clinic: The Clinic reserves the right to cancel the Agreement at any time and for any reason.
